Insurance only visits<\/strong>\u00a0are problem focused with shorter visit durations (typically 10 to 20 minutes long) that are required to meet the insurance companies guidelines based on medical necessity.\u00a0<\/p>

Membership visit<\/strong>\u00a0are extended in duration typically 30 to 60 minutes long, and provide personalized wellness plans with a strong emphasis on patient education, disease prevention, and performance based health care. Your insurance can only be billed for the portion of your visit that meets insurance guidelines.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/details>\n\t\t\t\t\t\t

Costs for In-Network<\/strong>\u00a0Insurance Covered Visits<\/strong><\/p>

Copay or Co-Insurance<\/p>

****if you know you have a deductible and know you have not met your deductible you can pay at the time of service if you do not want to be billed****<\/p>

Cost for Out-of-Network Insurance or Self Pay Visits<\/strong><\/p>

If you\u00a0do not<\/strong><\/u>\u00a0<\/strong>have Out-of-Network benefit coverage<\/u>\u00a0you will pay $175 for your 10-20 minute follow-up visit, or $599 for your 60 minute new patient visit.<\/p>

If you\u00a0have\u00a0<\/strong><\/u>Out-of-Network benefit coverage<\/u>\u00a0you will pay based on your plans benefits<\/p>

We will be billing insurance for patient with an out-of-network benefit. Your account will be updated according to your Explanation of Benefits that you will receive from your insurance carrier.<\/p>

Once we have received the\u00a0Explanation of Benefits<\/em>\u00a0from your insurance company your account will be adjusted according to your plan benefits.<\/p>

Tricare Prime without Referral<\/strong><\/p>

You will pay for your visits at time of services\u00a0without<\/em>\u00a0military discount because we are submitting to your insurance.<\/p>

We\u00a0WILL<\/strong>\u00a0submit to insurance, once your\u00a0Out-of-Network deductible has been met, your cost is typically 50% of visit charges for covered services.<\/p>

This deductible reset every calendar year.<\/p>

Patients who have insurance but do NOT want to use their insurance<\/strong><\/p>

Fees are the same, you will pay at time of services. Please make use the front desk knows you do not want to use your insurance for your visits and they will code your account accordingly.<\/p>

Patients without insurance<\/strong><\/p>

You will pay the self pay prices.<\/p>

Hormone Pellet Insertion Appointments<\/strong><\/p>

This is not an insurance covered service and fees will remain the same, and we will not be sending to your insurance carrier.<\/p>

Female Pellet Procedure $425\u00a0<\/p>

Male Pellet Procedure $750<\/p>

IV Therapy<\/strong>\u00a0is typically not a covered benefit of insurance unless it is deemed medically necessary. Since we are infusing compounded vitamins most insurance companies do NOT deem these types of nutrient infusions medically necessary.<\/p>

Some tests are covered by insurance, other specialty test or test deemed not medically necessary are not covered by insurance. We will alway discuss with you the cost of the testing being ordered.
